If you do not register with Vaxcare within 48 hours, your appointment will be automatically canceled.

5) FOR MODERNA 6mos-11y: Have your doctor fax a prescription to 317-981-1748 immediately after scheduling. I CANNOT ADMINISTER THE VACCINE WITHOUT A PRESCRIPTION.
